All your business and professional information is required to enter the partnership agreement. Complete the partner application with full details:

  • Practitioner name, title, contact details
  • License number, state of licensure, NPI
  • Intended product categories and volume estimate
  • Estimated: 10–15 minutes to complete

Upload Verification Documents

Ensure all credential documents are submitted to move your application forward.Upload your documents securely through our portal for compliance review:

  • Certificate of Insurance (COI) — mandatory
  • Active professional license — mandatory
  • NPI verification – if applicable
  • Business registration — for distributor applicants
  • Estimated: upload within 24 hours of application

Credential Review in Progress
