Important safety information

Phentermine / topiramate ER is a prescription medicine. It is not right for everyone: pregnancy — topiramate can cause birth defects; monthly pregnancy testing is required; glaucoma or overactive thyroid; use within 14 days of an MAO inhibitor; recent heart disease or stroke. Tell your doctor about every condition you have and every medicine and supplement you take.

Common side effects are described below. Stop and seek care for any severe or unexpected reaction. This page is education, not medical advice; your doctor and pharmacist give the instructions for your treatment.

Plain-language guide

Phentermine / topiramate ER, explained by our clinical team

What it is

Qsymia® combines a low dose of phentermine, an appetite suppressant, with topiramate, a seizure and migraine medicine that also reduces appetite. The combination allows lower doses of each than either alone.

Because phentermine is a Schedule IV controlled substance, prescribing follows federal and state rules for controlled medicines; our doctors explain what that means for your consultation.

FDA-approved uses

  • Chronic weight management in adults with a BMI of 30 or more, or 27 or more with a weight-related condition
  • Adolescents 12+ with obesity

Our doctors prescribe for approved uses. Any other use is a decision made only by your doctor, for you, after evaluation.

How it is taken

One capsule each morning, starting at the lowest strength and raised every two weeks as directed. It is best taken in the morning to avoid insomnia.

What to expect

Appetite reduction is usually noticeable within days. Common effects include tingling in the hands and feet, dry mouth, constipation, altered taste and trouble sleeping. Topiramate can affect thinking and mood in some people.

Who should not take it

  • Pregnancy — topiramate can cause birth defects; monthly pregnancy testing is required
  • Glaucoma or overactive thyroid
  • Use within 14 days of an MAO inhibitor
  • Recent heart disease or stroke
