Plain-language guide

Setmelanotide, explained by our clinical team

What it is

Setmelanotide treats rare genetic forms of obesity in which the brain's satiety signaling is broken. It restores activity at the MC4 receptor, the pathway those genetic variants interrupt.

This is a specialist medicine: a confirmed genetic diagnosis is required, and treatment is managed with the patient's genetics or obesity specialist. We list it for completeness and can help coordinate.

FDA-approved uses

  • Obesity caused by confirmed POMC, PCSK1 or LEPR deficiency
  • Obesity in Bardet-Biedl syndrome

Our doctors prescribe for approved uses. Any other use is a decision made only by your doctor, for you, after evaluation.

How it is taken

A daily injection under the skin, started at a low dose and raised over about two weeks under specialist supervision.

What to expect

Reduced hunger within weeks for responders. Darkening of skin and moles, injection-site reactions and nausea are common. Mood is monitored during treatment.

Who should not take it

  • Obesity without a confirmed genetic cause
  • Anyone without specialist genetic evaluation

Questions we hear

Can I get this for ordinary weight loss?

No. It is approved only for specific genetic conditions.

Why does skin darken?

The MC4 pathway shares signaling with pigment cells; the effect is expected and monitored.

How is the diagnosis confirmed?

Through genetic testing arranged by a specialist.
