What clinics need to open a professional account

The documents and checks EKO Labs uses to verify clinics and physicians before sharing professional pricing.

Professional accounts are for licensed healthcare providers and clinics. We verify every account before sharing pricing or accepting orders.

Documents we ask for

  • Business details: legal name, entity, address and website
  • Practitioner details: name, title and contact information
  • Active professional license, with license number and state
  • NPI number
  • Certificate of insurance (COI)
  • Business registration, for distributors and clinics

What we check

  • License status with the state board
  • NPI cross-reference
  • Insurance coverage on the COI
  • Multi-state details when a clinic operates in more than one state

How long it takes

Review usually takes 1–3 business days after we receive complete documents. Once approved, your tier (W1–W4) is assigned and pricing is shared.
